Newborn Twin Guide

Creating a Twin Feeding Schedule That Actually Works

A realistic guide to feeding newborn twins, supporting weight gain, and finding a rhythm that works for your family.

Important Disclaimer

This guide shares general information. Every baby is different. Premature babies, babies coming home from the NICU, and babies with medical concerns may need a specific feeding plan. Always follow your pediatrician's, lactation consultant's, or NICU team's recommendations.

Feeding newborn twins can feel like a full-time job. Add in pumping, burping, diaper changes, and trying to sleep yourself, and the days can quickly blur together.

Some families love a schedule. Some families feed on demand. Some families do a mix of both. I fed my twins on demand in the beginning, which was very demanding. Pun absolutely intended.

A feeding schedule does not have to mean strict rules. For many twin parents, it simply gives the day a loose rhythm and helps everyone remember who ate, when they ate, and what comes next.

Quick Navigation

Why a Feeding Schedule Helps

A feeding schedule can help bring some order to the newborn stage, especially when you are caring for two babies at once.

  • It helps babies get enough calories for healthy growth.
  • It makes it easier to remember who ate and when.
  • It creates small windows for parents to rest.
  • It allows partners, grandparents, or helpers to step in.
  • It reduces the feeling that someone is always eating.

The schedule exists to support your babies and your family. It should not become one more thing making you feel stressed.

How Often Newborn Twins Eat

Most newborn twins need to eat every two to three hours around the clock in the early weeks.

If your babies were born prematurely or recently came home from the NICU, they may need scheduled feeds every three hours even if they are sleeping. These frequent feeds are important for weight gain, hydration, and steady growth.

Your pediatrician will let you know when it is safe to let your babies sleep longer between feeds.

Feeding Together or One at a Time

There is no single right way to feed twins.

Some families prefer feeding both babies at the same time. Tandem breastfeeding or feeding two bottles together can save time once you get into a routine.

Other families find it much less stressful to feed one baby at a time. It does take longer, but it allows you to focus on one baby without feeling rushed.

When I first started breastfeeding my twins, I fed one baby at a time until I felt more confident. Once we all got the hang of breastfeeding, I gradually switched to tandem feeding. That worked well for us, but every family is different.

Some parents also choose to feed on demand instead of following a strict schedule. That can work too, as long as your babies are gaining weight well and your pediatrician is comfortable with the plan.

Sample 24-Hour Feeding Schedule

Example Schedule

  • 7:00 a.m. — Feed both babies
  • 10:00 a.m. — Feed both babies
  • 1:00 p.m. — Feed both babies
  • 4:00 p.m. — Feed both babies
  • 7:00 p.m. — Feed both babies
  • 10:00 p.m. — Feed both babies
  • 1:00 a.m. — Feed both babies
  • 4:00 a.m. — Feed both babies
  • 7:00 a.m. — Begin the next day

This is only an example. Some babies need to eat more often, especially if they are premature, slow to gain weight, cluster feeding, or following a feeding plan from their healthcare team.

If Your Twins Were Premature

Premature babies often have different feeding needs than full-term newborns.

  • They may need scheduled feeds even if they are sleeping.
  • They may take longer to finish bottles or breastfeeds.
  • They may need fortified breast milk or higher-calorie formula.
  • They may need frequent weight checks after going home.
  • They may have different feeding plans from each other.

It is completely normal if your twins are not ready to follow the same routine as full-term babies. Follow your care team's plan first.

Breastfeeding, Pumping, and Bottle Feeding

Every twin family feeds differently.

Some exclusively breastfeed. Some exclusively pump. Some use formula. Many families use a combination of all three.

There is no single right way to feed twins. The best feeding plan is the one that helps your babies grow while also supporting your family's well-being.

Tips That Make Feeding Easier

  • Keep bottles, pump parts, burp cloths, and diapers ready before feeding time.
  • Use a notebook, whiteboard, or app to track feeds.
  • Burp each baby well during and after feeds.
  • If one baby is usually slower, consider starting with that baby first.
  • Accept help whenever it is offered.
  • Stay flexible. Some days the schedule will work beautifully. Other days it may fall apart, and that is okay.

Remember

The goal is not a perfect schedule.

The goal is healthy babies, steady weight gain, and helping your family settle into a rhythm that works.

During the newborn stage, especially with premature twins, feeding is one of the most important jobs you have. One feeding at a time is enough.
